Filler complications

Filler complications and facial heaviness

Maybe your face feels heavy, puffy, a bit distorted. Maybe someone said you look tired, even after eight hours of sleep. Or maybe you just know. You look in the mirror and something is off. We hear this daily, not because it is rare, but because it is rarely assessed properly.

You are not imagining it

Most filler complications are not dramatic. They are subtle, slow and creeping. That is exactly why they get dismissed.

  • Puffiness under the eyes that does not settle.
  • Heaviness in the lower face, as though something is pulling down.
  • A face that has lost its definition rather than gained volume.
  • Skin that reacts more than it used to.

This is not always an allergy or a bad product. Often it is filler doing what filler does: absorbing water, migrating, and settling into places it was never meant to go.

2

Why it happens

Your face is in motion every day. Filler is not fixed. It moves, especially when placed near muscles, fat pads, or too close to the surface of the skin.

  • It migrates downward with gravity.
  • It can compress vessels, which reduces oxygen to the skin.
  • It distorts the natural contours of the face.
  • It adds false weight to the lower face.

What is most frustrating is what people get told. That it is just ageing. That they are imagining it. That they need more filler. None of those are the answer.

What we do differently

We do not guess. We scan.

  • High-resolution ultrasound maps exactly where filler has migrated.
  • We can see how it is affecting the structures around it.
  • We know what is filler and what is your own tissue before we treat anything.
  • We rely on anatomy rather than assumption.

This matters because dissolving blindly risks removing the wrong thing, or leaving the part that is actually causing the problem.

4

Your treatment options

Every plan is different, but most involve some combination of the following.

  • Ultrasound-guided dissolving, targeted at the exact location causing the issue rather than a general injection.
  • A structural reassessment once filler is dissolved, to see your natural shape again.
  • Volume restoration only if it is appropriate, and only in keeping with your anatomy.
  • Skin repair, because migrated filler often compromises circulation. This may include exosomes with electroporation, laser genesis for lymph flow, LED for inflammation, and radiofrequency or CO2 once inflammation is stable.

You do not need to feel dismissed or stuck in a cycle of overfilling. We do not add more. We unpack what is already there.

Questions, answered

We scan you. High-resolution ultrasound shows filler as distinct from your own tissue, including where it has moved to. Without that, anyone treating you is working from a guess about what was injected, where, and how long ago.

That is the fear most people arrive with. We dissolve only what is causing the problem, guided by the scan, rather than flooding an area. Once the migrated filler is gone we reassess your natural shape. Many people look less heavy rather than emptier.

It can be both, and that is why we assess rather than assume. Ageing does not usually cause puffiness that appears after filler and moves over time. If the scan shows filler where it should not be, that is not ageing.

Often yes. Migrated filler can compress the small vessels that supply the skin, which is why skin quality frequently improves once it is removed. We may support that with exosomes, LED and laser once inflammation has settled.

It is staged, not a single appointment. Assessment first, then dissolving in one or more sessions depending on how much filler there is and where it sits, then skin recovery. We rescan as we go so you can see what has changed.
